Sit down together in front of one screen, each with a sheet of paper and a pen. Notes go on paper, not into the app. The app runs the rest.
If your team keeps a shared place for notes (a wiki page, a board, a folder), keep the template there. Otherwise copy it from here.
You give feedback to help. Not to be right.
Name the concrete situation and say what the person can do differently, or keep doing.
Listen, do not justify. A thank-you is enough for now.
What you act on is your call. Thinking about it seriously is part of the deal.

Before you start: what became of the resolutions from last time? Look at your last note sheets or your shared notes.
Exchange briefly. This is not a progress report, more an honest look back. On your first Feedback Friday, simply skip this step.
Three quiet minutes each, notes on your sheet. Think about the other person:
Start with CONTINUE and name concrete situations. For STOP, also say what would help instead.
Just listen. Asking questions is fine, justifying is not. A thank-you at the end is enough.
Now you decide for yourselves: what do you take from the feedback, what do you deliberately set aside? Both are fine.
Keep your note sheets. Next Feedback Friday, the first thing you do is look back at your resolutions. And if you like, tell the team what you agreed on together. If not, that is fine too.
